Firehouse Center for the Arts Policies
Photo/Video Release
The Firehouse periodically photographs and/or takes video of the performances and processes that The Firehouse produces. These materials are used for publicity, marketing & archival purposes. Full names of participants will never be published without consent. This waiver grants The Firehouse permission to photograph, film, tape or record participant in any session in which they participate, and I waive all copyright and future considerations.
Release of Liability for Class Participation
- I agree to indemnify and hold harmless Firehouse Center For The Arts (The Firehouse), its Employees, Agents, Officers, Board Members, Volunteers or any other person against loss or expense, including attorney fees, due to any bodily injury, personal injury or property damage which may result from any and all activities while participating in classes or any other activity sponsored by or conducted by The Firehouse, or while visiting any facilities owned by, leased by, or controlled by The Firehouse.
- When using The Firehouse facilities, The Firehouse guarantees that all equipment and facilities are in good repair and are appropriate for the purposes to which they will be put, and personnel are appropriately trained and screened.
- It is understood and agreed that this Release of All Liabilities shall remain in force for not more than one year from the date of this Release. The release is recorded through a computer system, along with the date of agreement.
Youth Agreements
Participation
- I GRANT PERMISSION for my child to participate in all program activities with the Firehouse Center for the Arts (FCA), either on-site at a Firehouse facility or at a remote facility.
- If a student will be absent for any reason, I agree to email the director or leader asap.
- I UNDERSTAND that IN CASE OF ILLNESS my child may be asked to withdraw from participation; will be attended to The Firehouse Staff; and that I will be contacted as soon as practical. In the event of serious illness or emergency I understand that The Firehouse will immediately call 911.
- Students are asked to commit to the entire session, including all planned final public (group) performances.
General
- I UNDERSTAND that all electronic devices (cell phones, MP3 devices, video games, etc.) are to remain inside my child’s bag/backpack and NOT on their person. I understand that if my child is using any of these devices during the session they will be asked to surrender it until the session is over.
- I understand that The Firehouse will make every effort to have a secure area for particpant's personal items, but The Firehouse is not responsible for any lost or misplaced items.
- We ask that you follow the guidelines set out by most public schools to keep our programs peanut-free whenever possible. Note: The Firehouse is adjacent to a restaurant and cannot guarantee a strictly peanut-free environment.
- I UNDERSTAND that The Firehouse reserves the right to cancel a registration or to dismiss a child from programs/classes if attempts to remedy an unsatisfactory or unsafe situation or behavior cannot be resolved.
- I agree to provide the following for my child each day of the session:
- A peanut-free snack.
- A water bottle, clearly labeled.
- Safe, closed-toe shoes.
- A healthy, peanut-free lunch (for Full-Day programs only).
Refund Policy
Registrations cancelled at least two weeks prior to the start of the program are eligible for a full refund. Cancellations made after this date but prior to the start/audition date of the program are eligible for an account credit which may be applied to a program up to 12 months in the future. No refund or account credits are available once the program start and/or the auditions have been held.
